ANNOUNCEMENT TO ALL EXAMINEES OF THE LICENSURE EXAMINATION FOR PROFESSIONAL TEACHERS ON JANUARY 30, 2022 IN LUCENA CITY
Though Lucena City is in the Alert Level 3 Status, the Regional Task Force (RTF) of CALABAZON and Local Task Force (LTF) of Lucena City, after series of meetings and thorough deliberations, granted PRC Regional Office IV-A authority to proceed with the conduct of the Licensure Examination for Professional Teachers as scheduled.
However, examinees have to comply with the following requirements, as required by the RTF:
1. COVID-19 Vaccination Certificate (VaxCertPH) or an original vaccination card indicating that you are fully vaccinated;
2. Negative result of RT-PCR Test, valid within 72-hours from the time of testing; AND
3. Approved S-Pass (http://s-pass.ph/) Travel Coordination Permit for one day.

In addition to the above requirements, per Joint Administrative Order No. 2021-01 dated 28 July 2021, examinees shall present the following to the proctors:
1. Notice of Admission (NOA);
2. Duly accomplished Informed Consent Form (Annex “A”); and
3. Duly accomplished Health Declaration Form (Annex “B”).
Bring your own food and drinks as you are not allowed to buy them during break time.
Examinees are encouraged to come on the day of the examination and immediately return to their respective place of origin after the exam. Those who are arriving a day before the examination are required to stay at any of the DOT-accredited accommodation facilities.
