REMINDERS TO ALL EXAMINEES OF THE PROFESSIONAL TEACHERS LICENSURE EXAMINATION THIS MARCH 2022 IN LUCENA CITY
The examinees of the Licensure Examination for Professional Teachers are hereby informed that the conduct of said licensure examination will proceed as scheduled. As Lucena City is in Alert Level 1 Status, health and safety protocols shall still be observed and examinees are required to present the original and submit the photocopy of COVID-19 Vaccination Card (fully vaccinated).
Per Joint Administrative Order No. 2021-01 dated 28 July 2021, examinees shall present and submit the following to the proctors:
1. Notice of Admission (NOA);
2. Duly accomplished Informed Consent Form (Annex “A”); AND
3. Duly accomplished Health Declaration Form (Annex “B”);
In case there will be changes regarding Alert Level Status and health protocols as imposed by RTF and LTF, this office will post the necessary instructions through our Regional Office IVA - Lucena FB Page.
Bring your own food and drinks as you are not allowed to buy them during break time.
